Applicant, Thank you for your interest in becoming a member of the Nebraska CISM Program. The following link will open an application for membership PDF file. You may use this file to apply for membership to the CISM program in one of two convenient ways: 1) You may simply click on the link, wait for the file to open, print the file on your printer, fill in the respective fields, and mail the completed form to the address provided at the end of the application, or 2) You may click on the link, wait for the file to open, fill in each of the fields by typing in information (or selecting an option from the drop-down boxes), save the completed form to your computer's desktop with your last name as the file name, and then email the newly-created file to cism.coordinator@dhhs.ne.gov.
