CISM team members, Please note the new PDF versions of the CISM Team Activation Log and the CISM Activity Report Form. These files may be used in one of two convenient ways: 1) You may simply click on the link, wait for the file to open, print the file on your printer, fill in the respective fields, and mail the completed form to your regional CISM coordinator, or 2) You may click on the link, wait for the file to open, fill in each of the fields by typing in information (or selecting an option from the drop-down boxes), save the completed form to your computer's desktop with a unique name, and then email the newly-created file to your regional CISM coordinator via attachment to the email.
